The ASSURE Navigator Program

ASSURE Navigators are trained nurses* who can assist patients throughout the treatment journey via telephone. Navigators can assist patients with treatment scheduling, transition support, and help with remembering to take medication as prescribed. Additionally, they can connect your patients with locally available community support services, if desired.

ASSURE Navigators provide patients and their caregivers:

Connections to Local Care Centers

Creating Connections for Access to Care

ASSURE cares about the continuity of care for each patient. One way we are helping is by offering patients assistance in finding their preferred place of care to receive their treatment. Local Care Centers (LCC) are alternate locations (retail pharmacies, infusion centers, etc.) where patients can receive their prescribed injection from a licensed healthcare professional.

While ASSURE enrollment is not required to visit an LCC, your patient will receive comprehensive support by enrolling into the program, such as:

  • A phone confirmation of your patient’s completed injection or notification of a missed appointment
  • Patient appointment scheduling and reminder assistance with ASSURE Navigators
  • Improved access to treatment, which may include reimbursement support

Educate patients and their caregivers to help them have a more active role in the treatment journey

  • Educational resources on Continuity of care for you and your patients provided by your APM upon request
